Professional Improvement Program (PIP) Reimbursement Request Form

The PIP program is available to reimburse musicians for music-related costs.

This includes items such as; reeds, strings, instrument repair, instrument purchase, and mileage to Central Oregon Symphony rehearsals and performances.

Musicians are eligible for up to $100 each term (Fall, Winter, Spring) for a total of $300 for the academic year.

If a musician anticipates an expense above the available annual limit, they may request to carry forward any earned annual PIP reimbursements.

Forms and receipts are due to the Central Oregon Symphony Association (COSA) office by June 30, 2025.

Please submit the information below to process your reimbursement.

Checks will be processed after the Spring Concert Series and mailed in June.
